Give to UMKC form

Most online donations to UMKC are submitted using this form. The form had been updated by IT when the UMKC website was redesigned in 2008, but it had not been tested for usability or made functional on mobile devices. When an update to the content of the form came up, I suggested that we take the opportunity to redesign the layout as well. I made the page responsive, updated the fields and labels to current usability standards and improved the designation functionality. The previous version of the form required visitors to either type or copy a designation from a pop up and paste it into a comments field. This was counter-intuitive and led to many custom designations. I turned the designations into radio buttons accessed through a dropdown menu of categories, which made them both easier to find and choose.
